Staying motivated to succeed in whatever you set your mind to can be tricky at times. We seem to find ways to distract ourselves or find other "more important" or "interesting" activities that take us away from our true purpose.
Finding Your Passion
Turning your dreams into a reality requires passion. The life you dream of (relationships, family, health, spiritual, emotional, home and work), is obviously something you're passionate about - something you love and desire with all your heart. If it's not, then an honest reality check is in order.
Wanting something is not enough. You must hunger for it. Your motivation must be absolutely compelling in order to overcome the obstacles that will invariably come your way.

Staying Motivated
Keeping self motivated is a must if you want to succeed at your endeavors. To be successful you have to be able to follow through with your goals and that, in turn, requires self-motivation.
People who are unable to motivate themselves must be content with mediocrity, no matter how impressive their other talents.

Contrary to what you've read before, self motivation is something YOU have to do. In other words, no one else can do it for you. HOWEVER, paradoxically, you don't have to go completely solo. As a matter of fact, you need others to get it going.
Two Kinds of Self Motivation
The inner self motivation - also known as intrinsic motivation and the outer (exterior) or extrinsic motivation.
You'll need to have a healthy dose of both to succeed in converting your goals from a mere dream to a full fledged reality.
Assuming you are actually pursuing a life that excites you - then your motivation is internal - it comes from within - intrinsic motivation.
However, you will inevitably encounter set-backs, it's just part of life. For those times, you'll need to rely on outer motivational triggers - activities that will boost your self motivation or extrinsic motivation.
So, for those rough bumps when you'll need to count on external nudges here are some excellent self motivation tips to keep you on the path to success.
You can and must rely on outside stimulus or encouragement from activities or people to stay on track. Though, no one else can make the decision for you to self motivate, you will need some kind of outer support system in place.
Creating Your Support System
Creating a support system nurtures your self motivation. Surround yourself by people, things (books, movies, software tools, etc.) and environment (uncluttered workspace, calendars, planners, music, etc.) that will promote your personal and professional success or growth.
Remember, success is a journey, but you must follow certain road signs.

Spend Time with People You Admire
Don't just read about positive folks, hang-out with them. Become their friends, work with them or for them!
Spending time around self motivated and successful people will allow you to experience what makes them tick. Successful people are usually positive minded folks that have encouraging words for others. Take them in and apply what you see and hear to your life. They have already traveled part of the path to success. Learn from them!
Associate yourself with people of good quality, for it is better to be alone than in bad company.

Accountability Partner(s)
Part of your support system includes having an accountability partner(s) or "buddy". I can't over emphasize the importance of this!
Personally, I've had two goal setting partners that help me keep an eye on my page building and task completion goals for this site. I would probably slack off if it weren't for their constant encouragement and reminders.
This has also kept me humble in acknowledging that I can't do everything alone, I need others and others need me, as well.
Added bonus: The opportunity to give back by supporting your friends or business colleagues - it's gratifying and everyone wins!
Motivational Lives and Biographies
When you need motivation read about other successful people's lives, your role-models, through their biographies. People whom you admire and that inspire you to think: "If they can do that, so can I." These are men and women that already possess the qualities or abilities you want to acquire or are on the path of developing.
Read books or watch movies on the lives of great men and women, who overcame all kinds of obstacles, created an extraordinary life. I'm particularly drawn to those from the early 20th century, such as:
Elizabeth Towne, Wallace Wattles, Napoleon Hill, Orison Swett Marden, Oliver Wendell Holmes, Helen Keller, Anne Sullivan, Amelia Earhart, Coco Chanel, and James Allen, just to name a few.
When you are modeling someone's success, you should not just listen to what they say - you should also watch what they actually do.

Organizational and Motivational Tools
You'll definitely need a Planner like Franklin Covey or Day Timer. If you feel more comfortable with electronic devices like a blackberry or Palm, by all means, continue to use them.
Activities to Keep Your Self Motivation
Vision boards - creating physical and virtual software vision boards. Practice visualization exercises, self discovery exercises, or read and send out free inspirational ecards. Read or memorize daily affirmations, motivational and inspirational quotes.
